Salesforce Winter '17 Release: Health Cloud Updates

Health Cloud is getting smarter with Patient Management and supercharging the efficiency of connected professionals. With the Winter ’17 release, Salesforce is empowering care teams with two main featuresCare Plan templates and Telehealth, which are both intended to improve the patient journey.

Today, on the Provider side, care coordinators often spend significant amounts of time filling out paperwork to capture patient issues, prognosis and plans. This affects the whole experience of patient care, beginning with onboarding.

With a click of a button, the new Care Plan predefined templates allow care coordinators to add problems, goals and tasks in bulk to a patient’s care plan. You can create or purchase templates from leading content providers that can be customized and applied to meet a patient’s specific needs. Plus, Health Cloud will allow you to use Native Data Loader to import purchased templates. Loading predefined templates saves time for care coordinators and helps enforce standards of care that indirectly improve the care journey.

The snapshot below shows how to select a template to apply to patient, how to customize a care plan to meet a patient’s unique healthcare needs and how to review the care plan before you apply it to the patient record. After you click Apply, the care plan is added to Care Plan tab for the patient.

For providers, ER visits and re-admissions can be a costly proposition. The second feature I'd like to discuss from the Winter ’17 release will help address this problem. That feature is TeleHealth for Health Cloud.

This add-on feature lets patients connect with a care team in real time using two way video. Patients can access care at the push of a button from anywhere, by using their mobile device. This is win-win for both providers as well as patients.

Once the patient calls in, a care coordinator can video chat, screen share and answer any questions that can help avoid unnecessary in-person office visits. Since video is embedded in the Health Cloud console, care coordinators can use the patient data at their fingertips to give contextual advice. In short, Telehealth for Health Cloud enables patients to connect with the care they need in real time from anywhere. The Health Cloud product roadmap is looking very promising, as it plans to expand the existing data model to handle claims, eligibility and expanded clinical data. A number of other enhancements are earmarked around Health console (CTI, Wave Apps and Clinical Surveys), and you can probably expect to see a self-service Health Community feature next year. Of course, Safe Harbor applies.
